What Is Facial Balancing? Why More Filler Isn’t Always Better

Facial balancing is sometimes described online as putting filler into several areas of the face.

We don’t think that’s a particularly useful definition.

At SKM, facial balancing begins before a syringe is opened.

Your face is a three-dimensional structure.

The face isn’t a collection of isolated lips, cheeks, chins and jawlines.

Skin, fat compartments, muscles, ligaments, bone and blood vessels exist in an interconnected three-dimensional anatomy. Changes in one area influence how neighboring features are perceived.

Modern filler literature emphasizes exactly this point: safe and effective treatment depends on detailed knowledge of facial anatomy, tissue planes, product characteristics and appropriate injection technique.

That’s why simply asking, “Where should I put filler?” starts with the wrong question.

A better question is:

What is creating the imbalance I’m seeing?

Sometimes the answer is filler. Sometimes it isn’t.

A patient may notice a lower-face concern when the underlying issue involves proportions elsewhere.

Someone may ask for more cheek volume when the better decision is to leave their cheeks alone.

Someone may arrive asking for a particular number of syringes and discover that our recommendation is less.

That’s intentional.

Why anatomy matters

Dermal filler is a medical procedure.

The face contains an intricate vascular network, and although serious complications are uncommon, vascular occlusion and associated tissue injury are recognized risks. Rare severe complications can include visual loss. Understanding vascular anatomy and recognizing complications are therefore fundamental to safe filler practice.

This is one reason SKM doesn’t treat filler like a commodity.

We assess before we prescribe.

What are we actually trying to preserve?

This may be the most important question.

Good aesthetic medicine isn’t simply about identifying everything that could be changed.

It’s also about identifying what shouldn’t be changed.

Your expressions.

Your proportions.
