What Does Microneedling Actually Do to Your Skin?

The science of collagen induction, skin remodeling, and why one treatment may not be the whole plan.

Microneedling is controlled injury, and that’s the point.

Microneedling has become one of the most recognizable treatments in aesthetic medicine, but what happens beneath the surface is more interesting than the device itself.

During treatment, very fine needles create controlled microscopic injuries in the skin. Those injuries activate the body’s normal wound-healing response, involving inflammatory signaling followed by cellular activity and remodeling of the extracellular matrix.

This is why microneedling is also called percutaneous collagen induction therapy. Reviews of the scientific literature describe activation of the wound-healing cascade and subsequent collagen remodeling as central to the treatment’s mechanism.

What can microneedling improve?

Microneedling has been studied for concerns including skin texture, wrinkles, photoaging and acne scarring. A 2025 systematic review of facial rejuvenation studies found that wrinkles were the most commonly evaluated outcome, followed by texture, photoaging and laxity.

Evidence for acne scars is also meaningful. Systematic reviews of randomized trials have found microneedling to be a useful treatment option for atrophic acne scars, although treatment protocols vary considerably between studies.

Why doesn’t everyone need the same treatment?

Because skin isn’t standardized.

Age, skin quality, treatment history, pigmentation risk, scarring, goals and recovery all influence how a treatment plan should be constructed.

And importantly, much of the published facial rejuvenation literature evaluates multiple microneedling sessions rather than a single treatment. In the 2025 systematic review, 19 of 21 included studies used multiple sessions, although schedules varied.

That doesn’t mean everyone automatically needs three treatments. It means the science doesn’t support pretending there is one universal protocol.
